The ROHIT Method
A Five-Stage Clinical Reasoning Framework by Dr. Rohit, BHMS
The ROHIT method is an acronym and a sequential discipline developed to guide the clinician through every step of case comprehension: from initial human recognition to responsible posology and longitudinal tracking.
The Five Stages of the ROHIT Method
A structured, reproducible methodology guiding the physician from first recognition of the patient to long-term curative tracking.
Recognise: Context, Causation and Relevance
Recognise the patient, the presenting problem, the domestic and occupational context, aetiological triggers, and clinically relevant characteristics before categorizing complaints.
Map the human environment, historical departure from health (never well since), and family vulnerabilities within the initial five minutes.
- Identify the precipitating life shock or exposure
- Assess domestic, occupational, and environmental burdens
- Note constitutional background and past medical trajectory
The Five Sequential Disciplines
Each stage represents a distinct intellectual and phenomenological task in the clinical encounter.
Recognise: Context, Causation and Relevance
Before the physician can treat a sick person, they must recognise who they are beneath the presenting distress. Recognition perceives the human background: family dynamics, occupational burdens, domestic stresses, and the precise aetiological departure from health. What initiated the disturbance? Has the patient experienced grief, injury, or chemical exposure?
Observe: Sensory Perception of the Living Patient
Observation is the physician's sensory discipline. It requires noticing what the patient expresses without words: posture, muscle tension, gait, complexion, pupil reactivity, and breathing rhythm. Notice how the patient tells their story, what topics evoke emotional hesitation, and whether their physical demeanor confirms or contradicts their claims.

Holistic Integration: Synthesizing the Coherent Picture
Holistic integration brings disparate case fragments into an interlocking, coherent mosaic. Instead of leaving mental anxiety, digestive weakness, and joint stiffness in isolated mental compartments, the physician discovers the common biological thread uniting them. How does the emotional state influence physical thermals? How do circadian rhythms affect localized pain?
Individualise: The Characteristic Distinction
Individualisation isolates what is rare, striking, and peculiar to this patient, separating it from what is common and expected in the disease taxonomy. While diagnosis describes common tissue pathology, individualisation reveals the patient's unique biological signature: singular modalities, unexpected thermal preferences, and personal rhythms.
Treat and Track: Responsible Application and Disciplined Follow-Up
The therapeutic journey is not finished when the remedy is handed to the patient. Stage 5 demands responsible posology (matching potency and dose to vital susceptibility) and structured follow-up over weeks and months. The physician tracks energy, sleep, and emotional clarity alongside chief complaints, interpreting changes according to Hering's law and Kent's Twelve Observations.
